FALLUJAH, Iraq -- US and Iraqi forces took control of almost all of Fallujah yesterday, nearing military success even as pockets of resistance remained and relief agencies began moving to avert a humanitarian disaster in the city of roughly a quarter-million people west of Baghdad.
